Surveys find that physicians in hospitals and nursing homes disregard most advanced directives. Advanced directives can be any of the following legal documents that a patient writes to help with medical decision making when they cannot make decisions for themselves: living will, power of attorney, health care proxy, etc.

The Journal of the American Medical Association (JAMA) reports that physicians are unaware of the directives of three-quarters of all elderly patients admitted to a New York hospital.
